Early Childhood Screening
In order to enroll in kindergarten in a Minnesota public school, you will need to have your child screened. Early childhood screening will check your child's overall development in a one-hour appointment. It's recommended children be screened before their 4th birthday. Find an available appointment today!

• BHM Health & Emergency form
• BHM Health Emergency form - Español
• Minnesota Dept. of Health Potassium Iodide (KI)
• BHM Potassium Iodide (KI) Consent form
• BHM Potassium Iodide (KI) Consent form - Español
• Ethnic and Racial Demographic Designation form
• Up-to-date Immunization records
• Up-to-date Immunization records - Español
• Copy of child's birth certificate
• Early Childhood screening documents
